FEDERAL NEURO-PSYCHIATRIC HOSPITAL, BUDO-EGBA, KWARA STATE  
INVITATION TO TENDER FOR YEAR 2024 CAPITAL PROJECTS  
 This is to invite qualified and experienced contractors for the under-listed projects appropriated in its 2024 Appropriation Act for the Federal Neuro-Psychiatric Hospital, Budo-Egba, Kwara State.
   1.      SCOPE OF THE PROJECTS   2.      ELIGIBILITY REQUIREMENTS
  a.      Evidence of Certificate of Incorporation issued by the Corporate Affairs Commission (CAC) including Form CAC1.1 or CAC2 and CAC7.
 b.      Evidence of Company's Income Tax Clearance Certificate for the last three (3) years valid till 31st December 2024.
 c.      Evidence of Pension Clearance Certificate valid till 31st December 2024 (This requirement is applicable to bidders whose number of staff is 15 and above).
 d.      Evidence of Industrial Training Fund (ITF) compliance Certificate valid till 31st December 2024 (This requirement is applicable to bidders whose number of staff is 15 and above).
 e.      Evidence of Nigeria Social Insurance Trust Fund (NSITF) compliance Certificate valid till 31st December 2024.
 f.       Evidence of Registration on the National Database of Federal Contractors, Consultants and Service Providers by submission of Interim Registration Report (IRR)valid till 31st December 2024.
 g.      Affidavit duly signed by the deponent dated within the advertisement period: ·
        Disclosing whether any officer of the relevant committees of Federal Neuro-Psychiatric Hospital, Budo-Egba, Kwara State or Bureau of Public Procurement is a former or present director, shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars. ·        That no director has been convicted in any country for any criminal offence relating to fraud or financial impropriety or criminal misrepresentation or falsification of facts relating to any matter. ·        That the company is not in receivership, the subject of any form of insolvency or bankruptcy proceedings or the subject of any form of winding up petitions or proceedings. ·       
That the company does not have existing loan or financial liability with a bank, other financial institutions or third party.
 h.      Company's Audited Accounts for the last three (3) years 2021, 2022, & 2023. The seal and registration number of the Audit firm MUST be on the Audited report.
 i.       Reference letter addressed to the Medical Director, Federal Neuro-Psychiatric Hospital, Budo-Egba, Kwara State from a reputable commercial bank in Nigeria, indicating to provide credit facility for the project for the execution of the project when needed and dated within the advertisement period.
 j.       Company's Profile indicating its activities, with the Curriculum Vitae of key staff to be deployed for the project including copies of their Academic or Professional qualifications.
 k.      Verifiable documentary evidence of the last three (3) similar jobs executed in the last five (5) years including Letters of Awards and job completion certificates or valuation certificates for ongoing projects.
 l.       For Joint Venture/Partnership. Memorandum of Understanding (MOU) should be provided indicating among others the Lead Partner (CAC, Tax Clearance Certificate, Pension Clearance Certificate, ITF Compliance Certificate, NSITF Clearance Certificate,1RR & Sworn Affidavit are compulsory for each JV/partner).

 Note: All documents must be accompanied by a covering/forwarding letter on the company/firm's letterhead, including the phone number (preferably GSM) and email address. Directors' names and nationalities must be displayed on the Letterhead Paper. Please arrange and submit all documents/information in the sequence listed above.
3.      COLLECTION OF TENDER DOCUMENTS Interested Companies are to collect the Standard Bidding Document (SBD) from the Contact Officer, Medical Director’s Office of the Federal Neuro-Psychiatric Hospital, Budo-Egba, Kwara State from 9:00am to 3:30pm upon presenting evidence of payment of a non-refundable fee of ?10,000 only per Lot, payable to the FNPH Budo-Egba TSA account (remita account) and obtain official receipt indicating the LOT being bided for. Ensure you sign the Bid Collection Register.
   4.      SUBMISSION OF TENDER DOCUMENTS Prospective bidders are to submit tender documents in two (2) copies (Original and Copy) each for Technical and Financial Bids. TECHNICAL BID FOR LOT ...... AND FINANCIAL BID FOR LOT ......... and at the reverse side, the name of the bidding company addressed to: The Medical Director. Federal Neuro-Psychiatric Hospital, Budo-Egba, Kwara State Submission should not be later than 12:00 noon, Friday, 30th August 2024.
5.      OPENING OF TECHNICAL BIDS Only the technical bids will be opened immediately after the deadline for submission at 12:00 noon, Friday, 30th August 2024 at the Seminar Room of the Federal Neuro-Psychiatric Hospital, Budo-Egba, Kwara State in the presence of bidders or their representatives, while the financial bids will be opened immediately after the technical bids processing. Please ensure you drop bids in the dedicated tender box at the Contact office, as the Hospital would not be responsible for misplaced or wrongly submitted bids.
   6.      GENERAL INFORMATION
 a.      Bids submitted after the deadline for submission would be returned un-opened.
 b.      All costs will be borne by the bidders.
 c.      This advertisement is specifically for projects under the 2024 Appropriation Act and shall not be construed       as  a commitment on the part of the Hospital to award any or all the above listed projects.
 d.      The Technical Bids will be opened in the presence of Bidders or their representatives, CSOs in the areas of Anti-Corruption that chooses to join the bid opening.
 e.      Only qualified bidders at technical evaluation will be invited for financial bids opening while the financial bids of un-successful bidders will be returned un-opened.
 f.       Due diligence would be followed, and all documents submitted shall be verified.
 g.      Bidders should not bid for more than one (1) Lot.

 Federal Neuro-Psychiatric Hospital, Budo-Egba, Kwara State is not bound to pre-qualify and reserves the right to annul the procurement process at any time without incurring any liabilities in accordance with Section 28 of the Public Procurement Act.2007.
